I've got this pattern somewhere, will check tonight. I copied it out of a magazine back in the 80's. Its written for cotton but if you use the hook size for the yarn you want, it will work, will be larger than 4".
learn the chainless foundation row, wish I knew about it 40 years ago. It helps me determine how long my project is and I do it loosely so the first row doesn't end up being too tight
Magic knot is also wonderful, I'm not brave enough yet to cut the tails so I crochet over them as I go

there are openings for the strap the be inserted, blanket cannot fall off baby, that is the style I am looking for
